How to use Box 6
Include Box 6 with other federal withholding on Form 1040 line 25b. It is a payment toward tax, not the amount of Social Security that is taxable.
Example
With a $2,000 monthly benefit and a 10% Form W-4V election, SSA generally withholds $200 per payment and Box 6 accumulates the withholding for the year.

Withholding on Social Security is optional. Nothing is taken out of a benefit payment unless you ask for it, which is why the box is labelled voluntary and why a beneficiary with other untaxed income can reach filing season with no credits at all against the tax the benefit helped create.
You ask on Form W-4V, and the form allows only four choices. Federal income tax can be withheld from Social Security benefits at 7%, 10%, 12%, 22% of each payment — no other percentage, and no flat dollar amount. The request goes to the payer, not to the Internal Revenue Service; for Social Security that means the Social Security Administration, which also accepts the request through its online withholding page or by phone.
Any change needs a new form. Form W-4V states that the rate you choose stays in effect until you change it, stop it, or the payments stop. Raising or lowering the rate means a fresh Form W-4V with a different box checked, and stopping withholding entirely means completing the form again and checking the line that requests no further withholding.
Box 6 is a payment, not income. It joins the other federal income tax already paid for the year and is reported with withholding shown on Forms 1099, separate from the wage withholding reported from Forms W-2. Nothing about Box 6 affects how much of the benefit is taxable — that is decided entirely by Box 5 and the combined-income test.
The two figures can look contradictory and both still be right. A statement can show a substantial Box 6 alongside a Box 5 that produces no taxable benefit at all; the withholding is then simply refunded, and the sensible response is a new Form W-4V rather than a correction to the statement.
Sizing the rate is the harder question. A flat 7%, 10%, 12%, 22% applied to the benefit takes no account of pension income, individual retirement account withdrawals, or investment income, and Form W-4V itself notes that anyone with other income not subject to withholding should consider estimated tax payments as well. Where the benefit alone cannot carry the household’s tax, the shortfall is usually closed with estimated tax or with extra withholding elsewhere.

Is the withholding enough on its own?

Social Security benefits you repaid during the year, including amounts withheld to recover an overpayment.
Box 5: Net benefits
Box 3 benefits paid minus Box 4 benefits repaid. This is the amount used to determine how much Social Security enters gross income.
